? ?Position Summary:?The clinical pharmacy manager is responsible for coordinating clinical pharmacy services and ensuring that the pharmacy provides optimal clinical services on a daily basis.
The clinical pharmacy manager facilitates the development, implementation, and maintenance of systems that promote desirable patient outcomes through the use of safe, appropriate, cost-effective medication therapies.
??????????Essential Duties include but are not limited to:
o????Clinical/Management
??????????Coordinates activities of the Pharmacy and Therapeutics Committee including, but not limited to, preparation and distribution of the agenda, minutes, and other documents needed for the meetings.
??????????Ensures safe, appropriate, cost-effective medication therapies for patients according to established policies, procedures and protocols.
??????????Directs development, implementation and maintenance of clinical pharmacy services.
??????????Facilitates the development and implementation of treatment guidelines, protocols, formulary changes and critical pathways as approved by appropriate committees.
??????????Coordinates the selection and safe use of alternative medications in response to medication shortages.
??????????Facilitates the identification, resolution, implementation and communication of cost-effective medication-use issues in collaboration with the medical staff.
??????????Supports the pharmacy s pharmacoecomonic efforts.
Reviews the use of targeted medications for compliance with current pharmacoeconomic programs.
??????????Provides clinical consultation and clarification to practitioners.
Recommends evidence-based medication therapy regimens and monitoring plans.
Suggests appropriate, cost-effective therapeutic alternatives to medical staff as needed.
??????????Provides accurate, adequate and timely drug information to the professional staff.
??????????Publishes a pharmacy newsletter and/or contributes to facility newsletter for physicians and other health professionals.
??????????Provides training and education to pharmacists and other healthcare professionals regarding pharmaceutical care and pharmacoeconomics.
??????????Supervises and instructs pharmacists, residents, pharmacy technicians and students.
??????????Prepares and presents drug reviews, MUE criteria, antibiotic sensitivity reports, adverse drug reaction reports and pharmacy interventions summary to the P&T committee.
??????????Coordinates pharmacy related activities in accordance with the facility Antimicrobial Management Plan (AMP).
??????????Participates in the Antimicrobial Task Force.? Will perform reviews, carry initiatives and/or education as requested by the Task Force.
??????????Collaborates with laboratory in the development of an antibiogram annually.
??????????Reviews and evaluates the medical literature for studies supporting the pharmaceutical care and pharmacoeconomic programs.
??????????Participates in community educational programs (e.g.
diabetes classes, health fairs). ??????????Contributes to the quality and effective operation of the department.
??????????Reviews the performance and documentation of clinical activities of all pharmacists including documentation of the financial impact of clinical activities.
??????????Coordinates policies and procedures for clinical activities (e.g.
therapeutic interchange). ??????????Supports the improvement and accomplishment of corporate, division and facility objectives through interdepartmental teamwork and planning.
??????????Participates with the Director of Pharmacy in providing guidance to pharmacy staff regarding current Joint Commission, state and other regulatory standards compliance.
??????????Prepares reports on pharmaceutical care and pharmacoeconomic activities, including financial impact, as directed.
??????????Participates in the quality improvement activities of the department.
Collects and analyzes data and maintains documentation of all clinical activities.
??????????Establishes productive, collaborative relationships with staff members and within the community.
Voices concerns and suggestions to appropriate persons in a positive manner.
??????????Performs staff pharmacist duties when needed.
REQUIREMENTS



Position Requirements:


a.???????Licensure/Certification/Registration:?Appropriate, current state pharmacy license.
b.???????



Education:

?Bachelor s degree from an accredited college of pharmacy required, Pharm.D.
