As a member of the multidisciplinary care team, the RN or LPN provides general nursing care to residents.
The RN or LPN contributes to the overall resident care, which is guided by the resident care plan, facility policies and procedures, regulatory guidelines and professional standards of practice.
Plans, coordinates, and provides total patient care specific to diagnosis and age of patients served on a continuing basis, to obtain optimum health for patient and family, in accordance with accepted hospital standards and policy and the South Dakota Nurse Practice Act.
Demonstrates knowledge of standards, policies and procedures, and integrates knowledge into delivery of care.
Performs nursing procedures and treatments according to nursing division and unit specific standards.
Demonstrates the ability to appropriately assess patients, apply clinical interventions, evaluate outcomes, and implement change to the plan of care.
Formulates, implements, evaluates and revises plan of care based on assessment.
Reports to physicians and other staff patient response to plan of care.
Identifies and documents patient/family education needs.
Provides resources to meet their learning needs.
Prioritizes and organizes delivery of care in a consistent manner.
Initiates interdisciplinary collaboration to promote optimal patient care and outcomes.
Recognizes an emergency situation and initiates appropriate responses to prevent adverse event and promote positive clinical outcomes.
Utilizes critical thinking in implementing the coordination of medical and nursing interventions.
Interventions demonstrate care planning that will promote good clinical outcomes.
Adapts nursing procedures, medication, and treatment to meet needs of individual patient, routinely or on an emergency basis.
Administers medications and treatments properly and in a timely manner as documented on MAR.
Assesses pain when indicated. ?
Assess resident for possible and current skin issues with each scheduled skin assessment.
Implement any prevention measures needed to ensure residents skin integrity is maintained.
